3)  Community Tax Receipt (Cedula)
4)  TIN (Taxpayer's Identification Number)
5)  Photocopy of two valid !Ds (Passport,Driver's License, SSS ID, Company ID)
6)  Marriage Contract
7)  Proof of Billing


If borrower is employed:
1. Certificate of Employment and COmpensation ( Indicating Position and length of service)
2. Latest Income Tax Return ( ITR) of the borrower, duly stamped as received by the Bereau of International Revenue ( BIR ) or Certificate of Compensation Payment/ Tax Withheld
3. Pay slips for the last three months
4. Bank Statements for the last three months

If borrower is engaged in business:
1. DTI Registration if sole proprietor
2. Mayor's Permirt of Business Permit
3. Bank Statements for the last six months
